What do I need to bring the day of my procedure?
- Your insurance card(s).
- Driver’s License or Photo ID.
- A list of your current medications.
- Your procedure payment (the co-pay or deductible will be due at the time of service).
- If you are a self-paying patient and are paying for your procedure with cash, check or credit card, the full amount will be due on the day of your procedure.
- Leave valuables and jewelry at home.
- Wear comfortable, loose fitting clothes.
- Plan arrive at the center one and a half to two hours prior to the procedure. You should expect a stay of two to three hours total, depending on your procedure.
- If you’re younger than 18, you must have a parent or guardian accompany you to sign your paperwork. For children younger than 18, a parent/guardian must bring the child’s medical history and physical that was completed by a pediatrician. If you are an adult who is unable to sign the consent, you must have someone with you who is authorized to sign on your behalf.
- Bring the name and phone number of the person who will be taking you home (must be 16 years or older with a valid driver’s license). You will not be permitted to drive, take a taxi alone or leave the center without an adult chaperone.
Additional Information:
- Please do not eat or drink anything after midnight the night prior to your procedure, including water. If you have questions about whether or not you can take any of your medications the morning of surgery, please call (561) 374-5550 and ask for a nurse.
- If you experience any changes in your health prior to your surgery, please call and notify us immediately.
- You may want to bring a sweater or something to keep you warm in the waiting area as it is very chilly.
For Pediatric Patients
- Please bring the history and physical form written by your child’s pediatrician with you the day of surgery. Do not rely on this form being faxed to the center.
- Please bring your child’s favorite toy, blanket, pacifier, bottle and diapers, if applicable. Please remove any jewelry prior to arriving at the center.
- We are unable to accommodate more than two adults per patient. If at all possible, make arrangements for siblings to stay at home.
- Plan on one legal guardian to remain in the facility for the duration of your child’s stay with us.
